GOOD CATCH OF THE WEEK
An A/Z TM at the Lonza site in Portsmouth, NH noticed a sub storing an oxygen bottle and acetylene bottle together in a plastic hand carrier without proper separation. Storing compressed gas cylinders improperly—especially oxygen and acetylene—creates a serious fire and explosion hazard. These materials must be stored with appropriate separation to prevent dangerous reactions.
Great job to the team for staying alert and speaking up to keep the site safe.

CIANBRO SAFETY FLASH
While removing an elevated handrail, a TM loosened the left bolt, assuming both sides were secured. The handrail swung, pinching TM’s hand, causing soft tissue damage and a fractured thumb.
Though the removal process was adequate, not noticing an unsecured section led to a loss of control.
• Stand down of incident with all on site. • Discussion of incident with entire site; focus on LMRA and performing a thorough evaluation of material to be demolished.
