GOOD CATCH 

While on-site at the NCG Museum, a subcontractor was seen using the stair tower before the scaffolding tag was signed off. The TM was then educated on safety policies, including how the stair tower must be signed off by competent person before being accessed.

Lessons Learned

Defective Power Strip

A circuit breaker repeatedly tripped in a cubicle area due to a brand-new power strip that overheated when a laptop charger was plugged in. Resetting the breaker twice allowed an internal defect to overheat, causing the power strip to smolder and smoke. The breaker was then manually secured, and the power strip was safely unplugged. The power strip was ETL-listed and had surge protection.

Avoid using power strips when possible Use longer cords for direct outlet use Unplug power strips before resetting a breaker Defects may not be visible even in new equipment
