Think Before You Enter:
A fall hazard does not require a large height to cause a serious injury. The safest place during elevated work is within a properly protected work area. Always use approved fall restraint systems, including harnesses and SRLs, when required. –Has the Fall Protection Checklist been completed on the JSA? –Are red barricades and updated tags in place? –Is the scaffold tagged and inspected by a CP? –Has the Rescue Plan been reviewed?
Before beginning work at height, ensure scaffolds are properly set up, top and mid-rails and barriers are installed, and all personnel are protected from potential falls.
PNSY P-381 - Kittery, Maine
With the use of a crane, a Cianbro TM and a TMC TM were loading bundles of conduit onto a flatbed trailer. Once the conduit bundle was on the trailer, the TMs stepped onto the bundles to disconnect the rigging. This put the TMs over 6 feet off the ground, placing them in a fall hazard.
Major injuries can come from what you might think are the smallest hazards. Take the necessary precautions and stay aware to get home safe!
TM’s foot slipped on the frame of the trailer while stepping down from the Condux machine’s toolbox. The TM’s leg slid between the trailer frame and the attached toolbox, straining their knee as their body continued to fall backwards to the ground.

Root Cause
Insufficient access/egress plan. Project team did not identify the potential fall hazards associated with climbing down.
Corrective Actions
•Create a ladder and platform access/egress platform for rental Condux equipment. •Investigate alternative Wire Pulling rental equipment (Zeck puller).
